Z-wave outlet turn on on its own after scene shut it off

I have a z-wave outlet setup for many years with a scene to turn on and off a few times a day. Recently sometime after the scene run and it the outlet turn off it turn itself back on. In the log it just say used created trigger.

Any suggestion?
