Over my past couple of years of amassing Z-wave hardware, I’ve ended up with a handful of switches–both dimmer and relay–that will not include to my network. I’m not talking about “failed to go into learn mode,” I mean, I’ll exclude and Vera will “remove” it, and then I’ll attempt to include and nothing happens. All of the switches I have are either Leviton or Monster brand. I’ve purchased both new and used switches, and the rate at which this happens is around one in every eight or nine. In every case, I just marked the switch as “defective” and moved on.
Other than using them as a dumb switch, is there anything I can do with these? Are they salvageable? Is there a process by which I can make them work? This has happened over time, with both Vera 2 and Vera 3, on multiple versions of firmware and Z-wave networks in a couple of different homes.
As with a lot of Z-wave network wonkiness, I don’t expect that anyone has had this exact problem and found a solution… but I thought I’d ask.
I’d mostly like to know whether there’s any hope of recovering functionality of these switches. Anyone?
I’d try factory defaulting the switch:
Pull air gap for 5+ seconds by pressing the op of the switch until the bottom clicks out.
Push the bottom of the switch back in and then hold it until the LED turns amber and then red. This indicates the switch has been reset.
Then try including the switch again. If it won’t include after a reset, I’d assume it is a dead switch.

Z-Waver, for as long as I’ve been working with these Leviton switches, you’d think I would have known this… your advice put me on the right path. I was able to install three dimmers today that I’d written off and put into the junk box. Thanks!
